MEMORANDUM OF AGREEMENT (MOA)
AFMCI 90-103
Incidents and Investigations and Reports (25 May 2007)
1. American
Federation of Government Employees (AFGE) Council 214 and Air Force Materiel
Command (AFMC), hereafter referred to as the

Subject instruction implements AFI 44-102, Medical Care Management, AFI 50-503, Aerospace Accident Investigations, AFI
91-204, Safety Investigations and Reports,
and incorporates recent AFMC guidance on Command Staff Level reporting of
mishaps, accidents, and suicides. The parties accept a shared interest in the
proper investigation, analysis and reporting of incidents resulting in the
injury or death of Air Force employees. The parties jointly seek to identify
and prevent workplace factors, situations and circumstances that contribute to
such incidents. This agreement implements
AFMCI 90-103, subject to the following provisions:
a.
The local union
shall be notified of any incident resulting in the death or total disability
(Class A Mishap) of a represented employee. If local union wishes to obtain an
unclassified/sanitized copy of a Class A Mishap
report, a written request must be submitted through Center/Wing safety office
to the Air Force Safety Center at Kirtland AFB, NM.
b.
Suicide reviews
of represented employees shall be subject to the following conditions:
(1)
Only leadership
will be briefed regarding any suicide incident in order to impart lessons
learned to their employees.
(2)
The briefing
information will be sanitized to exclude personal information that is not
relevant to the cause of suicide.
(3)
All civilian
employee interviews must be voluntary.
(4)
The Parties
recognize they share no consensus on the scientific validity or significance of
the “four dimensions of wellness” as defined in the Suicide Review Template.
c.
The Suicide Event
Surveillance System database, referred to in Chapter 6 will be used for
military personnel only. No personal
information of BUE will be entered into this database.
d.
Suicide and
violence prevention training will be conducted in accordance with the MOA on
AFI 44-154, dated 26 June 2003.
3.
All remedies available under the MLA or 5
U.S.C. Chapter 71 are available to the Parties if either party believes the other
has failed to comply with any of the requirements of this MOA.
